Walter Koenig’s immediate cause of death was cardiopulmonary arrest, according to his death certificate, first reported by TMZ. The document lists chronic diastolic congestive heart failure as an underlying cause and acute renal failure as a significant contributing condition. Koenig died at age 90 on Sunday.
His agency announced his death in a Facebook statement on Monday, saying he died at home in North Hollywood of old age, with his daughter, Danielle, and niece by his side. The certificate also states that Koenig was cremated on October 30 and names Hollywood Forever Cemetery as his final place of disposition.
Koenig was best known for playing Pavel Chekov on “Star Trek: The Original Series.” He joined the show in its second season and later reprised the role in seven films.
William Shatner and George Takei, the two remaining living cast members from the original series, paid tribute to Koenig. Shatner recalled seeing his genuine affection for the “Star Trek” community at fan conventions. Takei said they had been friends for nearly six decades and described Koenig as a writer, teacher and loyal friend.
Koenig and his wife, Judy Levitt, had two children, Danielle and Andrew. Andrew died by suicide in 2010 at age 41. Levitt died in 2022, after the couple had been married since 1965.
